can be renewed

Documents Required:
For Age Under 18:

Original National Bank
Challan Receipt for Passport Fee
·
Children Registration
Cert-CRC (B-Form) with 2-copies
·
Photocopies of Parents’
National Identity Cards (CNICs)
·
Foreign Passport and its
copy (for Dual-Nationals only)
·
Applicant must reach
Passport Office with Father & Mother
For Age 18 & Above:
·
Original National Bank
Challan Receipt for Passport Fee
·
Original CNIC/ NICOP/
Smart-Card with 2- photocopies
·
Bring expired Passport
& copy to Passport Office to renew
Process / Steps:
1. Go
to Regional Passport Office, Customer Service Counter
2. Get
Orig. National Bank Challan Receipt for Passport Fee
3. Show
them original NIC/NICOP/Smart Card/CRC(B-Form)
4. Your
photograph will be taken and Token will be issued
5. On
your turn, staff will take your Fingerprints
6. Now,
Data Entry Operator takes data and gives its print
7. Read
printed Form carefully and put it below your signature
8. Collect
duplicate token from Data Entry Counter
9. Get
verification for Exit Control List-ECL & Black List-BL
10. You
have to pass / clear interview with Assistant Director
11. Child
below 18 years will get an attestation from Class-1 Govt.
12. Officer
on Printed Form, given by Data Entry Operator
13. On
Form, Officer will write his NIC, Contact No. with Stamp
Important Notes:
·
Children under age of 15
are issued 5 years’ passport only
·
5 years’ passport cannot
be converted into 10 years’
·
Children are issued their
separate passport
·
If the passport is lost/stolen, submit the copy of Police FIR
·
Passport lost 1st time, the double fee will be charged
·
Passport lost 2nd time,
4-times fee will be charged
·
Passport lost 3rd time, the case will be sent to Headquarters
·
